Appointments:
We strive to provide you timely and efficient care. We will routinely ask that you bring EKG reports, x-rays, and other diagnostic procedure results for your appointment. Failure to bring this documentation may result in our having to reschedule your appointment. Our surgeons rely on the results of these diagnostic procedures to determine whether surgery is required. Our surgeons see patients at three (3) locations and provide services at these locations on the following dates and times:

Payment Policy:
Insured Patients. Payment is due at the time of service according to the insurance plan. You should be prepared to pay any deductible and co-pay amounts at the time of the office visit.
Uninsured Patients. Payment is due at the time of service. Many procedures performed by our surgeons are expensive and we will gladly work with the patient on a payment plan if required. Our surgeons see many of our patients for the first time in the hospital, rather than the office. When this occurs we rely on the hospital to provide much of the patients information for our records. On the patients post operative follow-up visit to our office, we verify the information we have on file.

Cardiovascular Surgeons, PA recognizes the importance of
protecting the privacy of health information we collect while
providing treatment to you, and we are committed to protecting
your health information. The provisions of the Health
Insurance Portability and Accountability Act of 1996 (HIPAA)
require us to provide you with the uses and disclosures of
protected health information and your rights and our legal
duties with respect to your protected health information.
